Our trained Coaches will support you to make sustainable changes to what you eat and drink, how much and when, as well as your activity levels and lifestyle. Sessions are fun and engaging and we give you high-quality interactive resources.
Many services are for virtual (Zoom) or in-person groups, typically 15-20 people. The new NHS Type 2 Diabetes Path to Remission services are all individual (your choice of in-person or digital). You can only join the diabetes remission services, formerly the NHS Low Calorie Diet, with a GP referral for safety reasons as you may need to change your medications.

This GP-referral service for people recently diagnosed with Type 2 diabetes includes 21 individual coached ‘sessions’ over 12 months. Based on the DiRECT study this three phase programme includes 12 weeks of total diet replacement (shakes, soups etc.) Momenta is one of five organisations on NHS England’s national framework. The service is formerly known as the NHS Low Calorie Diet or ‘NHS soups and shakes’ programme. Delivery is to in-person groups of up to 15 participants in Somerset and North East & North Cumbria. In all other areas the service is individual (1:1), with your choice of in-person or digital formats.

Based on the DiRECT study at Newcastle and Glasgow universities, this three phase programme includes 12 weeks of total diet replacement (shakes, soups etc.) Delivery is to your choice of in-person or virtual groups of up to 15 participants.
This service supports eligible participants to reduce their blood pressure and risk of developing more serious conditions. You can be referred by your GP practice or the QE Chest Pain Clinic or self-refer if eligible. Designed with the NHS and BHF this programme has 18 x 1 hour sessions over 9 months. This service is funded by SBRI and is delivered in partnership with Birmingham and Solihull ICB, Birmingham City Council, West Midlands AHSN and other organisations.
You can self-refer or be referred to our weight management programme if you’re an eligible Birmingham City Council resident aged over 55. This programme has an initial getting Started session and 12 x 1 hour groups over 3 months. This service is commissioned by Birmingham City Council.
