High blood pressure puts you at increased risk of cardiovascular diseases (CVD) like heart attack or stroke and many other conditions like diabetes or kidney disease.
The London Borough of Barking and Dagenham (LBBD) is funding only 200 free places on a new programme to support people diagnosed with high blood pressure who are above a healthy weight - the Barking and Dagenham CVD Prevention Programme, or 'LBBD CPP' for short.
The LBBD CPP supports you to get all the knowledge, skills and tools you need to better control your blood pressure and make your heart fitter and stronger.
To qualify for a free place you need to start soon so please get in touch with your GP practice team quickly for a referral if you're interested and meet the criteria below.

The LBBD CPP has been developed by specialists working with NHS cardiologists, cardiac nurses, GPs and the British Heart Foundation. Early results show the programme can help people lose weight, improve their blood pressure and cholesterol, get fitter and feel healthier and happier.
There are 18 one-hour group sessions over 9 months. Each session focuses on a new behaviour change, nutrition or physical activity topic. Your trained Coach will discuss these topics with you and the group and help you think about how to put them into practice in your own life.
You'll be given a range of high quality resources including Workbooks, a Lifestyle logbook and 12-months' free access to EXi, a physical activity app. Due to Covid-guidelines groups are currently virtual using Zoom.

Relevant personal and medical information you provide to Momenta Newcastle (“Momenta”) will be stored securely on electronic systems and not used for any purpose other than programme delivery, ensuring patient safety and monitoring and evaluation.
The programme is delivered by Momenta, who will securely share this data and other programme-related data, including any test results and other outcomes, with your GP practice to enable the programme to be delivered safely and evaluated.
Momenta will also report anonymous descriptive and outcomes data (with no personally identifiable information) to the London Borough of Barking & Dagenham and the Department of Health and Social Care for evaluation and monitoring purposes.
Momenta will not share your information with third party marketing organisations.
Your data will be treated as confidential and held, shared and disposed of in line with all legal requirements (including the Data Protection Act) and NHS guidance (which includes the Caldicott Guidelines).
